Pilates and reflexology provide gentle yet powerful ways to ease common symptoms while boosting overall wellbeing. By focusing on posture and mindful movement practices, small changes (guided by safe techniques) can make the months of pregnancy a more positive and comfortable experience, writes Fiona Hardie.

Living life on the run is something most of us can relate to; however, taking time out to thank your body and celebrate its incredible power both anatomically and physiologically is so much more important than we can ever imagine and something we often don’t do until it is too late.

Taking this time out when you’re trying to start a family, are in the midst of a pregnancy, or have given birth a while ago, is non-negotiable when it comes to your mental, physical and emotional health, not to mention the health of your baby.

As a Pilates instructor and therapeutic reflexologist, I look at the importance of posture and relaxation before, during (in particular) and after pregnancy. At the same time, I hope to, in a tiny way, illustrate the interconnectedness of your body and its systems by linking the benefits of Pilates and reflexology.  

Pilates is not only a wonderful form of exercise, it’s also relaxing because of the focus on breath, which in itself is so beneficial for the unborn baby, as well as being calming and centring for you. Part of your postural health is also the health and well-being of your feet. Here, reflexology makes a great partner to Pilates during and after pregnancy. It is also fantastic where infertility is an issue because by creating balance and helping the body to heal, optimal conditions for conception are created.

How Pilates and reflexology work together

The focus of Pilates on posture is paramount in avoiding back, hip, neck and knee pain among a host of other niggles pregnant mommies-to-be often complain about. Any thorough preggie Pilates class (as well as a normal one) will dedicate some time to doing footwork. Footwork is so important because, with the increase in weight and change in posture, many expectant women end up with their feet growing in size and becoming flat-footed. The arches may drop with the additional kilograms, and since our feet are the foundation of our bodies, dropped arches can lead to other complaints such as ankle and knee pain, which can then lead to hip, lower back and even neck pain.

With 33 joints, 26 bones and a network of over 100 muscles, tendons and ligaments, and about 7,000 nerve endings in each foot, our feet are remarkable structures and deserve mega doses of TLC. There are 250,000 sweat glands in the feet, producing perspiration when active. Movement of the feet stimulates lymphatic vessels, which follow the veins, stimulating lymph drainage. Swollen feet and ankles are common maladies of pregnancy, and taking care of these masterpieces of engineering with Pilates and reflexology will benefit the entire body.

When we look at the condition of the feet, cracked heels are an unsightly, often painful, and very difficult symptom to get rid of. Despite numerous visits to beauty salons for pedicures, the cracks reappear within days, much to everyone’s despair. The reasons for these are many. Kidneys filter waste products and other toxins from the body. Poor diet, genetics and lifestyle choices can lead to a buildup of waste products. The kidney meridian also runs to the baby’s toes. Here, reflexology is wonderful for balancing the body’s systems and assisting in detoxing.

Another cause, and one where Pilates comes to the rescue, is a lower back imbalance. Strengthening the lower back while training the pelvis to maintain optimal alignment with the growing belly will keep this condition in check.

Let’s not forget stress levels. Any pregnant woman has some form of stress, from financial worries to fears of doing all the right things for her baby, as well as being pulled in a million different directions. Reflexology, with its powerful relaxation benefits, is top of the charts to help ease stress. Babies born to moms who enjoyed reflexology during pregnancy are known to be less fretful, sleep better and be much more relaxed themselves.

Incidentally, reflexology is brilliant for aiding breast milk production,  another incredibly important reason to take a load off your feet and treat yourself to a session.

Another common foot issue is plantar fasciitis, which is the tightening of the ligament running along the arch of the foot. This tightening can be due to tight calf muscles or shortened lower back muscles. From an emotional perspective, for a new mother suddenly needing to become the support system for her new baby and, in many cases, the rest of the family, this pressure can be an aggravating factor for plantar fasciitis. Never underestimate the link between emotions, thoughts and what is presented in the body. Bruce Lipton, Louise Hay and Deb Shapiro are all experts in this field and well worth looking up when there is a physical symptom that will not resolve itself,  despite all other interventions.

So many of the aches associated with pregnancy can be alleviated by using the body’s natural posture to support the numerous changes to the body. Here, Pilates comes into its own as a fantastic exercise modality and preventative measure. As the tummy grows, the typical backwards leaning pregnancy stance can impact the internal organs that are already being crowded by the growing foetus, thus causing all sorts of problems such as indigestion and constipation, shortness of breath and in turn fatigue.

Pilates and Reflexology: Gentle Ways to Ease Pregnancy Symptoms

A pregnant woman can call on her inherent skeletal alignment and deep core support to assist in holding her in a comfortable way that relieves many of the common complaints during the most challenging late months of pregnancy. With correct postural alignment, the core muscles function like a hammock, cradling the baby from below and holding the baby and tummy in place. Knowing how to sit, stand and bend is important for all human beings, but even more so for a woman who is doing the same for two. 

These same core muscles are integral during the actual birth process, too, helping to move the baby through a correctly aligned birth canal. A posture that is malaligned will activate all the wrong muscles, which will then interfere with the ability of the core muscles to push the baby out. It is for this reason that any exercises done during pregnancy must emphasise natural alignment as well as the core.

Combining the posture-enhancing and relaxation benefits of Pilates with the deeply relaxing and balancing effects of reflexology will make for a mom who is not overcome by back pain, heartburn, fatigue, swollen ankles, plantar fasciitis, anxiety… the list goes on. Instead, your baby will be born to a happy, chilled mom who, despite living life on the run, knows to make herself a priority to give her all to those she loves and adores more than anything in this world.

Tokophobia: What It’s Like to Fear Pregnancy and Childbirth

Tokophobia can feel like a constant shadow over a woman’s life. The fear of pregnancy or childbirth can be overwhelming, making decisions about starting a family stressful and emotionally exhausting. Many hide their anxiety, feeling alone, but understanding and support can help women regain control and hope.

It’s very common for women to feel anxious about labour and birth. Worries about the pain of contractions, interventions and the uncertainty of the process are not unusual. For some women, the fear of labour and birth can be so overwhelming that it overshadows their pregnancy and affects their daily functioning.

This severe fear of childbirth is called tokophobia, which literally means a phobia of childbirth. And for some women, this also includes a dislike or disgust with pregnancy.

Tokophobia can be split into two types: primary and secondary. Primary tokophobia occurs in women who have not given birth before. For these women, a fear of birth tends to come from traumatic experiences in their past, including sexual abuse. It can also be linked to witnessing a difficult birth or listening to stories, or watching programmes which portray birth as embarrassing or dangerous. Whereas women who suffer from secondary tokophobia tend to have had a previous traumatic birth experience, which has left them with a fear of giving birth again.

It is difficult to say how common tokophobia is. Research suggests that between 2.5% and 14% of women are affected by tokophobia. Some researchers believe this figure could be as high as 22%.

These figures vary so much because women with different levels of tokophobia were included in the research. So while some women may have relatively mild tokophobia, for others, the condition is much more severe. The figures may also include women who have anxiety and depression rather than tokophobia.

Not a happy occasion

Women with tokophobia come from a wide variety of backgrounds. It is difficult to predict who might be affected, although it is clear that women with tokophobia are also more likely to experience difficulties with anxiety and depression and other mental health problems.

Research suggests some women with the condition choose to avoid pregnancy altogether – or may consider a termination if they find themselves in that position. When pregnant, women with tokophobia may request a caesarean section to avoid the process of actually having to give birth.

Some women find pregnancy itself very difficult, particularly dealing with the growing bump and feeling the baby’s movements. Anxiety, insomnia, sleeplessness, eating disorders and antenatal depression or increased risk of postnatal depression, have all been identified as consequences of tokophobia.

Some of the consequences for women with tokophobia – which emerge during labour – are longer labours. These are usually with an epidural and increased need for forceps or ventouse – this is a cup-shaped suction device which is applied to the baby’s head to assist the birth. All of which can have implications for both the woman and her baby.

Afterwards, some women with tokophobia may have a less satisfying bond with their babies. A difficult experience of birth can make women more afraid of birth if they become pregnant again.

Tokophobia treatment

Anecdotal evidence indicates that clinical care for women with tokophobia is patchy. But the good news is that there is help out there for women with this condition. Some women find it helpful to talk through a previous experience of a traumatic birth, while others might be reassured by information about labour and birth. Other women, however, may need more targeted treatment;  many counselling approaches can be helpful.

Many women also find it helpful to visit the maternity ward and talk to midwives and obstetricians during pregnancy. Some women find the condition can be very isolating, feeling that nobody else shares this intense fear. For these women, simply knowing that they are not alone can be very comforting and helpful.

Overcoming a phobia

In Hull and East Riding of Yorkshire, where there is an established perinatal mental health service for women and their families, there has been a recognised need for a consistent approach to caring for and supporting women with tokophobia.

This has led to a group of practitioners, academics and patients to work together to explore the care and support available to these women – and to help address the gaps in service provision.

This pioneering work, which is at the forefront of tokophobia service provision and research in the UK, aims to ensure that women get the right support and that their psychological and pregnancy needs are met.

Tokophobia can have debilitating effects on women and their families. Some women will avoid pregnancy, even though they might want to have children. For those who do become pregnant, the condition can overshadow pregnancy and affect the choices they make for labour and birth. This is why we need to work towards preventing tokophobia if possible, as well as providing effective treatment for women who suffer from this difficult condition.

Tokophobia is an intense fear of pregnancy and childbirth. It goes beyond normal worries and can cause significant anxiety, distress or avoidance of pregnancy altogether.

Are there different types of tokophobia?

Yes.

Why Women Struggle to Access Early Antenatal Care in Pregnancy in South Africa

South Africa has extremely high maternal mortality levels. This is true when compared with developed countries as well as other developing countries.

According to the World Health Organisation, for every 100,000 live births in the country in 2015, 138 women died due to pregnancy and childbirth complications. In Sweden, fewer than five women die for every 100,000 live births. In Brazil, the estimate is 44 women for every 100,000 live births.

Many of these deaths are preventable. And in South Africa – unlike many other developing countries – women mostly give birth in health-care facilities and visit antenatal clinics before they give birth. So why are the maternal mortality rates so high?

Earlier research has shown that many women attend antenatal care for the first time too late in their pregnancies. This is due to:

  • overburdened clinics, resulting in many being turned away without help;

  • bad treatment by health-care workers, resulting in women devaluing the service;

  • their fear of testing HIV positive and the associated stigma; and

  • Their cultural beliefs – that they may be bewitched by jealous neighbours or friends.

But our research shows that many women visit antenatal care clinics late because they discover they are pregnant late in their pregnancies.

Why women avoid antenatal clinics

Antenatal care services are mostly provided at community clinics that also provide a variety of general and primary health-care services to the community. These include care for HIV, TB and lifestyle diseases. These clinics tend to be busy and the staff are often overburdened and overworked. All patients, including pregnant women, often have to wait for a long time before being seen by a nurse or doctor.

We interviewed women at public health-care facilities, including hospitals and birth facilities, in an urban area in the Western Cape province shortly after they had given birth.

More than a quarter of the women interviewed went to the clinic late into their pregnancies. Women who went to the clinic for the first time when they were five months pregnant were classified as attending a clinic late. But if attending an antenatal clinic at or after three months of pregnancy was considered late, attending after the first trimester is usually considered late; almost four in five women said they went late.

More than half of the women who said they sought antenatal care late did not go to the clinic early enough because they realised quite late that they were pregnant. Some said they purposefully postponed the visit. And more than two-thirds of the women who went late said nothing about the clinic would have made them go earlier.

The study showed that women who went late were more likely than those who went early to not have completed high school. Women who went late were also more likely to be poor. And compared with those women who attended clinics early into their pregnancies, these women were also more likely to say they consumed alcohol while they were pregnant.

We did not find evidence to show that late attendance was related to travel costs to access antenatal care at free public clinics.

But the relationship between poverty, education and general vulnerable circumstances is complex. While our research shows that women who attend antenatal care clinics later are likely to be poorer and have less education than women who attend early, we cannot say this is necessarily true for all poor or less-educated women. But poverty and lower education levels may make it more difficult for people to make the best choices about when to seek health care.

Why are early antenatal visits are important?

The World Health Organisation recommends at least four visits to an antenatal clinic during pregnancy. Although South African women almost meet this target, their late attendance has an impact on the ability of the health-care system to influence their health and that of their babies.

Attending an antenatal clinic early in pregnancy is important for two reasons.

First, if pregnant women attend the clinics in the first three months of their pregnancy, HIV can be detected early, and they can begin treatment. This makes it less likely that their babies will contract HIV. It also helps to support their own immune systems, which decreases the chance of infections before or after birth.

HIV is the biggest contributor to maternal deaths in South Africa. Almost a third of pregnant women who visit antenatal clinics in South Africa are HIV-positive.

Second, early attendance allows doctors to treat and manage other treatable health conditions that the mother-to-be may develop. These include high blood pressure and anaemia, which are also major risk factors for maternal deaths.

How pregnancy policy must change

If late pregnancy recognition is one of the major reasons for late attendance, what does this mean for policy about pregnancy care in South Africa?

There are two policy changes that could rectify this.

First, the National Department of Health needs to make it easier for women to recognise pregnancy. Urine pregnancy tests should be as widely available as condoms.

If women realised that they were pregnant sooner, it would improve early antenatal care attendance and enable them to make more informed choices. For example, they would have time to consider an abortion in the case of an unplanned pregnancy.

Second, the way contraception is provided at clinics has to be reconsidered. Four in five women in our study said they had an unplanned pregnancy. The current system, where women can receive contraceptives at clinics, does not have the intended effect of preventing pregnancies. This could be due to the large focus on HIV prevention, which may limit some of the policy attention provided to contraception.

If this system were changed, women would be able to make better choices for themselves and their children.

Implementing these measures could go a long way to improve maternal health and ensure fewer children grow up without their mothers.The Conversation

SA Medical Experts Respond to Trump’s Paracetamol Claims in Pregnancy

Many women were left alarmed following United States President Donald Trump's recent assertion that paracetamol usage during pregnancy may be linked to autism. This statement has raised concerns among medical professionals worldwide, prompting a swift response from South African leading medical associations, namely the Society of Obstetric Medicine South Africa (SOOMSA), the South African Society for Ultrasound in Obstetrics and Gynaecology (SASUOG) and the South African Society of Obstetricians and Gynaecologists (SASOG).

South African medical associations have stepped in to set the record straight after United States (US) President Donald Trump claimed that pregnant women should avoid taking Tylenol (acetaminophen, known as paracetamol in many countries, including South Africa). During a press conference at the White House on 22 September 2025, Trump warned that taking Tylenol during pregnancy could increase the risk of autism spectrum disorder (ASD) in children.

The Society of Obstetric Medicine South Africa (SOOMSA), the South African Society for Ultrasound in Obstetrics and Gynaecology (SASUOG) and the South African Society of Obstetricians and Gynaecologists (SASOG) released a joint statement, giving a clear message that aims to reassure expectant mothers and healthcare providers about this widely used painkiller.

Trump’s Warning and the Scientific Context

Trump said, “Don’t take Tylenol,” arguing that the common painkiller may increase the risk of autism in children when used during pregnancy. 

While his warning drew attention, experts quickly pointed out that the evidence does not back him up.

Most studies on the topic are observational, meaning they can detect associations but cannot prove causation. Researchers have long cautioned that other factors (like genetics, pre-existing health conditions, or lifestyle) may explain why children develop autism, not paracetamol itself.

For example, a Swedish sibling-comparison study found that once family and genetic background were taken into account, the suggested link between paracetamol use and autism virtually disappeared. In other words, there’s no solid scientific proof that the medicine causes autism.

South African Medical Bodies Respond

In South Africa, the three leading organisations (SOOMSA, SASUOG and SASOG) came together to dispel Trump’s suggestion.

In their joint statement, they said:

“In agreement with position statements from several international bodies, we confirm that there is no evidence that taking paracetamol in pregnancy at recommended therapeutic doses causes autism in children.”

Referencing the Swedish study, the groups acknowledged that earlier studies had reported links, but they emphasised that these were weak and likely due to distortion. Their position was firm: “Comprehensive systematic reviews and meta-analyses confirm this conclusion: once unmeasured confounding is considered, there is no meaningful increase in ASD risk. Together, the evidence strongly suggests that earlier reported associations were explained by confounding rather than a causal effect of paracetamol.”

The medical societies also underlined that paracetamol is still the first choice for pregnant women who need to treat pain or fever.

“Paracetamol remains the safest and most effective first-line treatment for fever and pain in pregnancy,” the statement read.

Risks of Avoiding Treatment

The organisations also warned about the dangers of avoiding paracetamol altogether because of misinformation. Untreated fever in pregnancy has been linked to serious outcomes, including miscarriage, congenital malformations, preterm labour and even stillbirth.

As the statement explained: “Fever in pregnancy is associated with increased risks of miscarriage, congenital anomalies, preterm birth and fetal demise.”

They also pointed out that leaving severe pain untreated can have ripple effects on maternal mental health, which in turn affects both mother and child. “Untreated pain during pregnancy has a significant impact on maternal mental well-being and pregnancy outcomes,” the associations noted.

What This Means for Pregnant Women

The bottom line from South Africa’s leading maternal health experts is that pregnant women should not be alarmed by Trump’s remarks. They can continue to use paracetamol if needed, but always within the recommended dosage and ideally under the advice of their healthcare provider. 

The societies were clear in their guidance to healthcare professionals: “There should be no change in current clinical practice regarding the use of paracetamol in pregnancy.”

Medical bodies stressed the importance of balance. Avoiding treatment based on unverified claims could pose more risk than benefit. Pregnant women are encouraged to discuss any concerns with their doctor rather than relying on sensational headlines

When you’re expecting, you’re constantly told what’s “bad” for your baby. Many mums give up alcohol and cigarettes immediately, but vaping is often advertised as the lesser evil. Since it doesn’t have the smell or the stigma of smoking, some mums-to-be convince themselves it’s a harmless option.

The Chemicals Lurking Inside Vape Liquids

Here’s the reality: vape liquids are far from harmless. They contain:

  • Support groups for pregnant women
  • Mindfulness and relaxation techniques
  • Professional counselling

It’s important to have support instead of going it alone.

Organisations like the World Health Organization (WHO) and the Royal College of Obstetricians and Gynaecologists warn against vaping during pregnancy. Studies consistently show risks to both mother and child, stressing that no form of nicotine exposure is safe.

If you’re vaping and pregnant, honesty is the best policy. Your healthcare team won’t judge you; instead, they’ll guide you towards safer choices. Remember, their only goal is to protect your baby and your well-being.
