World Doula Week (22–28 March) is observed globally. There is growing awareness around a role that is quietly reshaping how many families experience pregnancy, birth and the early days of parenthood, the doula.

Also known as a birth companion, a doula is a trained, non-clinical professional who provides continuous emotional, physical and informational support throughout pregnancy, labour and the postpartum period. While doctors and midwives are responsible for clinical care, doulas focus on the overall experience, ensuring that a mother feels supported, informed and reassured every step of the way.

In many ways, this support fills an important gap.

Pregnancy and birth are not only medical events; they are deeply personal experiences that require time, understanding and consistent care. In busy healthcare environments, continuous one-on-one support is not always possible. This is where birth companions play a meaningful role, offering a steady presence from early pregnancy through to the postpartum period.

During pregnancy, doulas help families prepare for what lies ahead. This includes sharing information about the stages of labour, discussing birth plan options and helping parents feel more confident in making decisions that align with their preferences.

When labour begins, doulas provide ongoing emotional reassurance and physical support through techniques such as movement, positioning, and counter pressure. They also support partners, helping them feel more confident and involved.

Doulas work across all birth settings, hospital, home or birth centre locations, providing continuity of care regardless of where the birth takes place.

Support continues into the postpartum period, where families adjust to life with a newborn. Doulas offer guidance, reassurance and practical support during this transition.

In South Africa, awareness is growing and more families are reaching out to certified doulas. Access is also expanding, with a range of doulas offering different approaches and styles of care.
